To help you prepare your funding application, the MDSSC has prepared several documents. You can download them here or email us at legacyfund@mdssc.ca and request a copy.
You can use the templates provided or use them as a guide to prepare your own submission. Either way, it is important to answer all the questions and provide all the information requested.
Application Package – this provides you with all the instructions you need to submit your application for funding. It includes guidelines and examples of the application templates.
Application Form – a fillable application form you can download and use to prepare your funding request.
Budget template – a budget template must be included with your application. You can download and fill out this template or prepare your own.
Sample support letter – if you’re not sure what your support letter should say, here’s a template you can use.
